Profile and waste forms
An ERS Waste Profile Form must be submitted for all manifested wastes.
An ERS Material Profile Form must be submitted for all non-RCRA regulated materials
that will be shipped to Phibro-Tech on a bill of lading.
When submitting a profile, please attach a copy of all MSDS forms associated with the material to
be recycled. For mixtures, several MSDS forms may be required. The MSDS information is also needed
to complete the profile accurately and is a great aid to Phibro-Tech’s lab staff when analyzing
the sample.
